Donation of Securities Forms
A gift of publicly traded securities can provide you with an unexpected means to make a significant contribution to Western at a relatively low-cost to you. You and your broker can electronically transfer these types of gifts into Westerns account by using our forms below.
The Process
1. Forward the completed transfer form to your broker/investment advisor to initiate your electronic transfer of securities. Western cannot initiate this transfer on your behalf.
2. Forward a copy of the completed transfer form to Western for our records.
3. Please be advised that in normal circumstances, the value of the donation receipt will be based on the closing price of the security on the day the securities are received into Western’s brokerage account, in accordance with our Board Policy and Canada Revenue Agency Regulations.
4. Unexpected and /or unidentified transfers make it difficult to issue appropriate tax receipts

Document Instructions
- Click on your required bank form
- Save document to your computer, do not edit the document in your internet browser
- You can either print the form and fill out by hand or fill out the form digitally by following the instructions below
- Open in Adobe Reader or Acrobat
- Fill in all applicable fields
- Print form, if you have issues with printing your document, see troubleshooting below
Troubleshooting
If your printer prints out a blank form or blank page try one of the following:
- Go to file>>print, a dialog box will display. Select "Print in grayscale (black and white)" and print the document again
- Go to file>>print, a dialog box will display. Click the advanced button, in the popup box select: "Print As Image" and print the document again
- If you still experience issues check if your adobe reader is up to date by going to "Help" in the top menu and select "Check for Updates" if your software is not up-to-date, update your software and try again
